Community Inclusion Services
Community Inclusion services (CIS) are provided outside of the individual’s residences that support and assist individual in educational, enrichment or recreational activities. CIS services are delivered in a group setting not to exceed six (6) individuals. *Please note that examples are not all inclusive of everything that can be funded through this service, small group outings to community festivals, museums, book clubs, theater groups, cultural events, holiday celebrations, sporting events, etc.
Individual Supports
Individual Supports (IS) are services that provide direct support of one-on-one and assistance for individuals, with or without the caregiver present, in or out of the individual’s residence, to achieve or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community. Individual Supports may include but are not limited to assistance with community-based activities, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, accompanying individuals to doctor appointments, prepping and cooking meals, assisting the individual with performing light housekeeping duties, running errands, providing companionship and activities of daily living. The services provided within the home & in the community.
Supported Employment – Individual & Small Group Employment Support
Support Employment are activities needed to help assist individuals obtain and maintain an individual job in competitive or customized employment, or self-employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary. The service may be delivered for an intensive period upon the individual’s initial employment to support the individual, because of their disability, would not be able to sustain employment without support. Support in the intensive period is delivered in a face-to-face setting, one-on-one. The service may also be delivered to an individual on a less intensive, ongoing basis where support is delivered either face-to-face or by phone with the individual and/or his or her employer. Services are individualized and may include but are not limited to: job development, discovery, training and systematic instruction, job coaching, benefit support, travel training, and other workplace support services including services not specifically related to job-skill training that enable the individual to be successful in integrating into the job setting.
Prevocational Training
Prevocational services will provide learning and work experiences, including volunteer work, where the individual can develop general, non-job-task-specific strengths and skills that contribute to employability in paid employment in integrated community settings. Services may include training in effective communication with supervisors, co-workers, and customers; generally accepted community workplace conduct and dress; ability to follow directions; ability to attend to tasks; workplace problem solving skills and strategies; and general workplace safety and mobility training. Prevocational Training is intended to be a service that individual’s receive over a defined period of time and with specific outcomes to be achieved in preparation for securing competitive, integrated employment in the community for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities. Support is delivered in a face-to-face setting, either one-on-one with the individual or in a group of two to eight individuals.
Career planning
Aiding with individuals to obtain, maintain or advance in competitive employment or self-employment. focused, time limited service engaging a participant in identifying a career direction and developing a plan for achieving competitive, integrated employment at or above the state’s minimum wage. R.I.T.C will assist with finding other competitive employment more consistent with the person’s skills and interests or to explore advancement opportunities in his or her chosen career. Services will be unique and catered to the individual.
Cognitive Rehabilitation (Supports Program)
A systematic, functionally-oriented service of therapeutic cognitive activities, based on an assessment and understanding of the person’s brain behavior deficits. Services are directed to achieve functional changes: by reinforcing, strengthening or re-establishing previously learned patterns of behavior, or establishing new patterns of cognitive activity or compensatory mechanisms for impaired neurological systems. Therapeutic interventions include but are not limited to direct retraining, use of compensatory strategies, use of cognitive orthotics and prostheses. Activity type and frequency are determined by assessment of the participant, the development of a treatment plan based on recognized deficits, and periodic reassessments. Cognitive therapy can be provided in the individual’s home or community settings.
Behavior Supports
Behavior Supports are provided to an individual and/or group counseling, behavioral interventions, diagnostic evaluations, or consultations related to the individual’s developmental disability and necessary for the individual to acquire or maintain appropriate interactions. A complete assessment of the challenging behavior(s) will be completed, development of a structured behavioral modification plan, implementation of the plan, ongoing training and supervision from R.I.TC DCC (Direct Care counselor’s) & SRC’S (Situation Response Counselor) and periodic reassessment of the plan when necessary.
Natural Supports Training
Training and counseling services for individuals who provide unpaid support, training, companionship, or supervision to participants. For purposes of this service, individual is defined as: “any person, family member, neighbor, friend, companion, or co-worker who provides uncompensated care, training, guidance, companionship or support to a participant.” Training includes instruction about treatment regimens and other services included in the Service Plan, use of equipment specified in the Service Plan, and includes updates as necessary to safely maintain the participant at home. Counseling will be aimed at assisting the unpaid caregiver in meeting the needs of the participant.
Interpreter Services
Interpreter service delivered to an individual face-to-face to support them in integrating more fully with community-based activities or employment. For language interpretation, the interpreter service must be delivered by an individual proficient in reading and speaking in the language in which the individual speaks. Interpreter services may be delivered to an individual’s home or in a community setting. Interpreter Services provider will develop strategies to assist the individual in reaching the desired outcome(s).
Community Base Supports
Community Based Supports (CBS) are provided by R.I.T.C by one of our Direct Care Counselor’s (DCC) to provide one-on-one support to the individual. Services can be provided with or without the caregiver present, in or out of the individual’s residence, to achieve and/or maintain the outcomes to increase independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan created to develop independence in the community/in-home activities. Support may increase assistance in the community as well as training and supervision of, individual as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
Respite
Services provided to individuals who are unable to care for themselves independently because of the absence or need for relief of those people who normally provide care for the individuals. Respite services may be provided in the participant’s home & in the community depending on the circumstances.
Supports Brokerage
Support Brokerage Service/function that assists the individual (or the individual’s family or representative, as appropriate) in arranging for, directing, and managing services. Serving as the agent of the individual or family, the service is available to assist in identifying immediate and long-term needs, developing options to meet those needs, and accessing identified supports and services. Practical skills training is offered to enable families and individual to independently direct and manage program services. Examples of skills training include providing information on recruiting and hiring personal care
Assistive Technology
Utilizing devices and equipment that can help individuals perform activities of daily living and other tasks. Assistive Technology is used to increase, maintain, or improve functional capabilities of participants.
Transportation
Service offered to enable individuals to gain access to services, activities, and resources, as specified by the Service Plan.
